The Cancer Detection Rate and its Cost of Screening Breast Ultrasound Performed in Community-based Hospital

Abstract
Purpose: To evaluate the additional detection rate of breast cancer and its cost of screening breast ultrasound in asymptomatic women having negative mammograms.

Materials and Methods: From June 2008 to May 2009, we enrolled 1,811 women having normal or benign findings of mammogram and additional screening ultrasound in a community-based hospital. We categorized the sonographic findings according to ACR BIRADS. We analyzed screening ultrasound-detected and histologically confirmed breast cancers and its cost for detection of an additional breast cancer on screening ultrasound.

Results: Sonographic findings were assessed as category 1, 41.0%; category 2, 34.8%; category 3, 22.3%; category 4, 1.9%. US-guided core biopsies were performed in 2.5% (n=10) of category 3 lesions and 80.0% (n=28) of category 4 lesions. There were 2 breast cancers among category 3 and 4 lesions (0.2% and 2.9%, respectively). Positive biopsy rate was 5.3% and the detection rate of additional cancer was 0.1%. The cost of screening breast ultrasound was 228,186,000won.

Conclusion: For asymptomatic women having negative mammograms, the detection rate of additional breast cancer on screening ultrasound was 0.1% in community-based hospital. The cost for additional detection of one breast cancer on screening ultrasound was 114,093,000won.
